Insurance
This practice does not participate in any insurance network, including Medicare and Medicaid. It is out of network with all plans. Payment is made to the practice directly at the time of the appointment.
All patients are treated as self-pay patients and have the right to a written good faith estimate of expected charges, described below.
Fees
The full rate schedule is sent to you in writing before a first appointment is scheduled, so you have every figure in advance. Payment is due at the time of the appointment. The practice accepts payment from your bank via ACH, and also payment by credit card.
Two commitments worth stating here rather than burying in a rate schedule:
There is no charge for obtaining your records. The practice contacts your prior clinicians and pursues the documents. Third-party copying fees levied by other institutions are passed through at cost and disclosed before we proceed.
If fees change, your existing rates hold for six months after written notice. This applies to every patient of the practice.
Your right to a good faith estimate
This applies to all patients of the practice, none of whom bill insurance.
Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill for medical items and services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate.
